Disability ACT is now accepting application forms to the ACT Companion Card Scheme.

ACT Companion Card Application Form
If however you are a member of the ACT Taxi Subsidy Scheme we can use some of the information you have previously supplied under the Taxi Subsidy Scheme for your application to the ACT Companion Card Scheme. In signing the permission form you will provide us consent to access this information and you will only need to fill out the sections indicated in the permission form.
Please Note: Your membership of the ACT Taxi Subsidy Scheme does not automatically provide you with membership eligibility to the ACT Companion Card Scheme, as Scheme membership is based on evidence of the need for life long attendant care.
